CLINICAL AND FUNCTIONAL EFFICACY OF SGLT2 INHIBITORS IN PATIENTS WITH CHRONIC HEART FAILURE AND PREDIABETES: RESULTS OF A PROSPECTIVE COMPARATIVE STUDY

Abstract

Chronic heart failure (CHF) is frequently associated with metabolic disorders, including prediabetes, which contributes to insulin resistance and progression of cardiovascular complications. SGLT2 inhibitors have demonstrated cardioprotective effects; however, their impact in patients with concomitant CHF and prediabetes requires further investigation. Aim.
To evaluate the effects of SGLT2 inhibitors on clinical, functional, echocardiographic, and metabolic parameters in patients with CHF and prediabetes. Materials and Methods. The study included 53 patients with NYHA class II–III CHF. The main group consisted of 28 patients with CHF and prediabetes who received standard therapy combined with an SGLT2 inhibitor. The control group included 25 CHF patients without carbohydrate metabolism disorders. Clinical status, ECG, echocardiographic parameters, blood pressure, and metabolic indicators were assessed at baseline and after 6 months of therapy. Results. SGLT2 inhibitor therapy was associated with a reduction in dyspnea severity and improvement in functional status. Left ventricular ejection fraction increased from 41.8±5.6% to 47.2±6.1% (p<0.001), while systolic blood pressure decreased from 142.5±12.3 to 128.7±10.4 mmHg (p<0.001). Favorable changes were also observed in metabolic parameters, including reductions in HbA1c and HOMA-IR index (p<0.001). Conclusion. In patients with CHF and prediabetes, SGLT2 inhibitors improve clinical symptoms, cardiac functional parameters, and metabolic profile. These findings support the potential role of SGLT2 inhibitors as an effective component of comprehensive therapy in this high-risk patient population.
